Before Your First Appointment

A thorough hearing evaluation includes a number of tests to determine your hearing ability, but equally important is the time we spend getting to know you and your needs. When you arrive for your visit, we’ll ask you to complete a new patient form that reviews your medical history, including information about any injuries, trauma, or exposure to loud sounds. Certain medications can also contribute to hearing loss, so it’s essential that we have all of your prescription information on hand.

We encourage patients to bring a loved one with them to their appointment, as it’s helpful to have the support of a friend or family member during this initial visit.
